Engine hesitation while maintaining a speed.

Default Engine hesitation while maintaining a speed.
I have a 2003 grand marquis with 90,000 miles, when I am maintaining a speed I feel a jerking sensation. I thought it might be old spark plugs and an old fuel filter so I changed both of them out. But I am still feeling the same thing. Does anyone know what the problem is?

Is it just around one speed or at any speed?

Maybe a throttle positioning sensor dirty/worn?
